Calvary Flora McDonald Retirement Community is appointing a Clinical Care Coordinator to oversee clinical care plans and AN-ACC funding assessments.

Calvary Health Care is seeking an experienced Registered Nurse to join our residential aged care home in Adelaide as Clinical Care Coordinator. You will lead the clinical team, oversee comprehensive resident care planning, and ensure compliance with Aged Care Quality Standards.

Key Duties & Core Responsibilities

In this critical role, your operational duties and key performance responsibilities will encompass:

• Coordinate comprehensive: Coordinate comprehensive clinical assessments and individualized care plans for 90 residential elders.
• Conduct AN-ACC: Conduct AN-ACC funding tool assessments and maintain accurate clinical documentation.
• Supervise medication: Supervise medication administration, wound care protocols, and infection control standards.
• Lead and: Lead and coach Registered Nurses, Enrolled Nurses, and personal care assistants.

Required Qualifications, Licenses & Experience

To be successful in this position at Calvary Health Care, candidates are expected to satisfy the following essential selection criteria:

• Mandatory Qualification: Bachelor of Nursing & AHPRA Registration.
• Relevant Experience: Minimum 3-5 Years of verified industry experience in Australia or recognized overseas equivalence.
• Current unconditional registration with AHPRA as a Registered Nurse.
• Minimum 3 years clinical experience in residential aged care or community gerontology.
• Solid working knowledge of the Aged Care Quality and Safety Commission standards.
• Clear National NDIS Worker Screening Check and Australian Police Clearance.

• Competitive Remuneration: AU$95,000 – AU$110,000 per year + standard Australian Superannuation (11.5%).
• Salary $95,000 – $110,000 + 11.5% super + not-for-profit salary packaging.
